8. Form Four Index Number (e.g. S1234-0001)
9. Year of Form Four Graduation
10. Name of Secondary school
11. Name of Primary school
12. Mobile Phone Number
13. Program of Choice (select from the dropdown)

B. Guardian/Parent Information

1. Full Name of Guardian/Parent
2. Relationship
3. Adress
4. District
5. Region
6. Guardian/Parent's Phone Number

Step 3: Preview and Confirm

1. After filling the form, click “Preview” to review your details.
2. You may choose to Edit or to submit and Proceed with Payment.

Step 4: Pay Application Fee

Pay a non-refundable application fee of TZS 20,000 via mobile money:

Step 5: Application Status
